Who is at Fault?

There is no point to find who's at fault when things happened. What is the point? And people are pushing "fault" around. Hey u, 35% fault, u 25.156%, u 21 %, u 2%, u 16.844%, making up a total of 100%. There is nothing as 1% fault, 99% fault. Once you are at fault, even 0.00001% also consider as wrong.

The best way is to think how to prevent it from happening agian, which i think is more practical, rather then pointing at others and cover your backside. If needed, sit down and think of a standards or SOP, so that if things ever happen a gain, we can follow the procedures and avoid this similar incident from happening. Isnt this better?

We should really just treat this as a lesson.
